Essential Functions
Direct Care (100%)
- Provide education, referrals and case management to clients
- Assist clients with rental assistance, housing linkage and other financial capability issues
- Develop action plans with clients to help them meet their goals
- Completes and maintains clients records and accurately and timely documents all client interaction
- Facilitate and conduct group rehabilitation
- Engages with our community Partners
- Encourages Clients to express their feelings and discuss what is happening in their lives, while helping them to develop insight into themselves and their relationships
- Provides support to individuals, to assist in overcoming dependencies, adjusting to life, and working towards defined recovery goals.
Qualifications
- High School Diploma or GED
- Associates Degree ,Field of Study: Human Services or 3 Years of Related Experience
- Bachelor's Degree, Field of study: Human Services 3 Years of Related Experience Preferred.
- CCS experience and Certified, preferred but not required
- Valid Driver License
Employee must be able to pass a criminal background check and be free of offenses prohibiting employment under HSF12 at the time of employment, and annually thereafter.
