Duties
- Provide behavioral therapy and crisis intervention to individuals in need
- Conduct individual and group counseling sessions
- Collaborate with primary care nurse and providers
- Coordinate or make aware of social services resources, i.e., housing, clothing, food, mental health services, etc.
- Collaborate with other social workers
- Conduct Case Management
- Work with hospitals for discharge planning
