Required Document: Current driving record. Please log onto the DMV website and request a copy of your driving record and scan in and attach it to your application. First Aid certificates are REQUIRED. Please scan and attach the certificates to your application before the deadline. Applications without valid First Aid certificates will not be considered. Please note that BLS does not count as First Aid. First Aid must be specifically listed on the card/certificate. Online certifications are only accepted if they are obtained through a blended learning class, which is composed of an online portion and an instructor-led skills session, which is taught in person. Certificates must meet guidelines of the American Red Cross or American Heart Association.
