What are the responsibilities and job description for the MAMMOGRAPHY PATIENT CARE NAVIGATOR (FT; 40hrs/wk) - Temple Health/Jeanes Hospital position at Temple Health – Temple University Health System?
The Patient Care Navigator functions to provide access to quality care for the women's center/mammography. In conjunction with the mammography management team, the Patient Care Navigator builds trusting relationships with patients through guidance, engagement, coordination of care, resource allocation, and assists with questions. The Patient Care Navigator serves as a point of contact to patients assisting with support. The Patient Care Navigator must demonstrate a model of service, team collaborator, and must be willing to interact with other co-workers and patient's caregivers. This role includes staying current with organizational resources and available community resources for the patient population the organization serves. Develops and maintains relationships with key referring physicians. Act as a liaison between the Radiologists and referring providers to expedite patient care; including obtaining scripts/pre-certs in advance of scheduled procedure. Works with Lead Interpreting physicians and ensure all TUHS Mammography departments are compliant with MQSA regulations and ACR requirements. Assists in maintaining biopsy results auditing and statistics, and in the daily operations of the Mammography Departments.
Education
High School Diploma or Equivalent (Required)
Bachelors Degree (Preferred)
Other : Completion of Patient Care Navigator education program (Preferred)
Experience
2 Years experience in a healthcare setting or equivalent preferably in mammography (Required)
General Experience And Knowledge Of Basic Clinical Concepts (Required)
General Experience and knowledge of EPIC systems (Preferred)
General Experience and knowledge of complex patient issues and social determinants of health (Preferred)
General Experience and knowledge of patient outreach methods and strategies (Preferred)
General Experience With Medical Terminology (Preferred)
General Experience working as a patient navigator/community health worker (Preferred)
