What are the responsibilities and job description for the BHCM / SOCIAL WORKER (FT; 40HRS / WK) - TEMPLE HEALTH / EPISCOPAL HOSPITAL position at Temple Health?
To provide ongoing continuity of care to adult psychiatric patients. These duties include discharge planning, family contacts, linkages and relationship building with aftercare agencies and contact with community supports. Integral member of the interdiscipinary treatment team. BHCM / SW's are also responsible for timely utilization review of all patients. This includes concurrent and retrospective reviews with various managed care organizations.
Education
Masters Degree : Accredited School of Social Work (Required)
Experience
1 Year experience as a Social Worker in a Hospital setting. (Preferred)
1 Year experience in Mental Health. (Preferred)
1 Year experience in Utilization review. (Preferred)
General Experience in Bi-lingual skills. (Preferred)
License / Certifications
CPR - Cardiopulmonary Resuscitation (Preferred)
SW-LIC - PA Social Worker License (Preferred)
