JOB DESCRIPTION: SOCIAL WORKER

Tenants and Owners Development Corporation (TODCO) is a community-based housing/community development corporation for San Francisco's South of Market Neighborhood. Our summary Mission is South of Market Neighborhood Builders. The TODCO Group of non-profit companies planned and developed new and rehabilitated affordable housing for seniors, persons with disabilities, and SRO households, with eight operating residences housing almost 1,200 residents. TODCO provides extensive resident services and programs through our Resident Services/Activities Program (RSAP) and engages in a wide range of community development programs and advocacy with other South of Market community organizations.

Position Title: SOCIAL WORKER

Position Summary: Under the direction of the Social Work Supervisor, the Social Worker is responsible for assisting residents living in TODCO properties with housing stabilization and retention by providing counseling and support services, case management, service coordination, community outreach, advocacy, information, and referrals to existing off-site providers. The position is full-time (40 hours per week), in-person/on-site, exempt.

Reports to: Social Work Supervisor

Schedule: Monday – Friday, with occasional Weekends and/or Evenings as scheduled

Salary: $85,000 - $105,000 Annual (100% Full Time Equivalent) Depending on Qualifications & Experience

ESSENTIAL DUTIES

§ Deliver social work services, case management, and support services programs for residents

§ Manage a caseload of individuals

§ Provide individual and family counseling

§ Proactively outreach to all residents to promote optimal engagement and wellness

§ Proactively identify tenants in need of services and maintain awareness of tenant's overall health status, including but not limited to specific medical, mental health and substance use issues

§ Complete comprehensive intakes and assessments with tenants at move in

§ Conduct home visits

§ Arrange and facilitate family and case conferences

§ Complete reassessments, and care plans and update face sheets

§ Complete progress notes in one business day

§ Provide residents with information and referrals to off-site service providers in the community.

§ Provide supportive counseling and case management services to ensure the successful transition of clients to community-based resources.

§ Provide crisis intervention, conflict resolution.

§ Organize and facilitate community-building activities including but not limited to: Food Pantry, cultural celebrations, large and small group social events, holiday celebrations, therapeutic groups, skill building workshops, and informational workshops.

§ Serve as liaison/advocate for tenants with Property Management and off-site service providers.

§ Attend all scheduled meetings with RSAP and Property Management staff.

§ Attend all regularly scheduled meetings.

§ Attend all on-site Property Management facilitated Resident Meetings.

§ Develop new resources and relationships with outside service providers.

§ Attend monthly off-site community meetings as assigned.

§ Comply with all mandated reporting requirements, including Adult Protective Services and HSH.

§ Comply with all HIPAA and PHI policies and procedures.

§ Maintain accurate and up-to-date records and files on all tenants receiving services using database.

§ Maintain and provide accurate statistical records, reports, and documentation of services as required by internal and external agencies as applicable.

§ Document and monitor tenant intakes and assessments.

§ Participate in the coordination of site programming and delivery of activities with activities staff.

§ Other duties as assigned.

REQUIRED SKILLS

§ Experience working with / knowledge of and sensitivity to issues impacting the senior, disabled, and marginalized populations.

§ Strong analytical, assessment and counseling skills.

§ Ability to handle a variety of tasks simultaneously.

§ Initiative and creativity.

§ Computer proficiency: Microsoft Word and Excel.

§ Demonstrated capacity to work with culturally diverse, low-income populations.

§ Ability to maintain strict confidentiality.

§ Strong leadership skills.

§ Attention to detail.

§ Excellent crisis intervention skills.

§ Ability to work as a member of a multi-disciplinary service team.

§ Excellent knowledge of substance abuse, mental health issues.

§ Excellent benefit advocacy skills.

§ Excellent problem-solving skills.

§ Strong communication skills.

MINIMUM QUALIFICATIONS

§ MSW strongly preferred.

§ Three years experience working with low-income people in a social service setting.

§ Three years of experience working with adults and/or elderly populations.

§ Demonstrated knowledge and understanding of substance abuse and mental health issues and their effects on individuals, elderly, and immigrant populations.

§ Demonstrated knowledge and understanding of issues related to aging in place.

§ Knowledge of current resources and service/benefits programs in San Francisco and the Greater Bay Area.

§ Ability to pass a background check.

§ Must be fully vaccinated and follow current CDC recommendations for COVID-19.

PREFERRED QUALIFICATIONS

§ Bilingual English / Spanish or Cantonese language desired, but not required.

§ Experience working in a Permanent Supportive Housing setting.
