MISSION STATEMENT:

Tender at Home is dedicated to providing clients with quality non-skilled care through trust, communication, empowerment and support that will stimulate their motivation and their mental desire to heal.

POSITION SUMMARY:

Tender at Home is looking for a Home Care Aide Lead who is moved by providing personal care to clients in their home. We are not looking for temporary hires, but rather an individual who wants to find a career in an agency focused on changing home care. The Home Care Aide Lead will work closely with the Home Care Case Manager to provide quality services for our clients.

POSITION QUALIFICATIONS:

  • Home Care Aide certification required, as obtained through successful completion of an approved program
  • Has one full year of lead supervision with caregivers
  • Has one year full-time experience within the last five years in direct patient care in the home care setting; OR Has one year-full-time experience as a CNA or HHA in an institutional setting, such as a hospital or nursing home,
  • Attends Aide oriented in services
  • Participates in professional meetings when directed
  • Able to work closely supervised to ensure competence in providing client care
  • Home care aide certification through DSS

PHYSICAL REQUIREMENTS:

  • Visual/hearing ability sufficient to comprehend written/verbal communication.
  • Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
  • Ability to deal effectively with stress.
  • Able to work a minimum of 40 hours per week.
  • Able to bend and stand an average of 6 hours per day.
  • Able to lift up to 50-75 pounds.
  • Able to write up to 3 hours per day.
  • Able to work in a stressful environment.
  • Able to drive 50-100 miles per day.
  • Able to assess and communicate with ill patients, co-workers, and general public.
  • Is neat in appearance and practice, with good personal hygiene.

COMPTENCY:

The content of the competency evaluation of the agency will include is not limited to:

  • Communication skills
  • Observation, reporting, and documentation of a client’s status and the care or service furnished
  • Basic infection control procedures and instruction on universal precautions
  • Basic elements of body functions and changes in body function that must be reported to the supervisor
  • Maintenance of a clean, healthy, and safe environment
  • Knowledge of emergency procedures and emergency protocol
  • Recognition of the physical, emotional, and developmental needs of a client
  • Ability to work with the populations served by the agency, including the need for respect of client privacy and property
  • Appropriate and safe techniques in home care services including but not limited to:
  • Bathing
  • Exercising
  • Making telephone calls
  • Meal preparation and planning
  • Dressing
  • Positioning
  • Transferring
  • Feeding
  • Companionship
  • Housekeeping
  • Laundry
  • Personal hygiene and grooming
  • Transportation
  • Shopping
  • Toileting and Incontinence Care

DUTIES:

Supervision of home care aides

  • Completes people development plans for home care aides
  • Orients home care aides
  • Ensures home care aides meet credential requirements
  • Obtains any reports or issues from home care aides
  • Works with Director of Home Care to improve home care aide procedures

Care Planning

  • Ensure quality and safe delivery of home care services
  • Participates in development and implementation of client plans of care per home care agency policy and procedure, as appropriate
  • Participates in client case conferences according to home care agency policy and procedure, as appropriate
  • Ensures the provided home care services reflect client plans of care
  • Regularly reports information regarding client plans of care is submitted to the home care case manager in a timely manner

Reporting and Documentation

  • Client plans of care are discussed with the home care case manager on regular basis
  • Client records are documented per home care agency policy and procedure
  • Client assignments and reports are received from home care case manager
  • Documentation is submitted within agency turnaround times
  • Participates in professional meetings when directed

Training

  • Able to work closely supervised to ensure competence in providing client care

  • Demonstrates ability to read, write, and carry out directions
  • Shows evidence of maturity and ability to deal effectively with job demands
  • Possesses good verbal and written communications skills
  • Remains professional in all aspects

Job Type: Full-time

Pay: $42,000.00 - $50,000.00 per year
