Key Responsibilities:
As a Physical Therapist at Tender Home Health, you will:
- Provide exceptional patient care services at the patient’s home, helping them heal comfortably and effectively.
- Collaborate with physicians, agency staff, and contract services to coordinate and implement personalized Plans of Care.
- Document patient services, care coordination, and collaboration with other healthcare professionals, ensuring all documentation meets home health standards and agency policies.
- Review patient caseload charts every 60 days to ensure compliance and assess patient progress, making necessary adjustments to the care plan as needed.
Required:
- Active California Physical Therapist License
- Ability to travel to patients’ homes within the coverage area
- Experience in public health, home care, home health care, or hospital settings
- Strong communication, presentation, interpersonal, analytical, and problem-solving skills
- Bilingual skills are a plus
