Position Title: Structured Family Care
Department: Clinical
Reporting To: Assistant Director of Nursing/or Clinical Manager
Job Statement: Structured Family Caregiving is a caregiving arrangement in which a patient lives with the caregiver, unless grandfathered in prior to February 1, 2020, or the caregiver is providing respite care. The caregiver provides daily care and support to the patient based on the patient’s daily care needs. The main goal in this role is to provide safe and professional care to our patients in their homes.

Duties may include but are not limited to:
- Assist the patient with activities of daily living (ADL’s) such as personal hygiene, eating, dressing, shaving, bathing, toileting, or showering etc.
- Assist with recording pertinent information pertaining to the patients care.
- Planning and preparing meals according to specific dietary requirements of the client, and feeding the client, as needed.
- Assist with ambulation with or without mechanical aids.
- Observing the appearance of gross behavioral changes in the patient and reporting to the RN case manager or other members of the Clinical Department.
- Maintain patient care areas in a clean, safe environment, adequate for the provision of care.
- Provides medication reminders and assists the patient to take medications as prescribed.
- Transfer patients in a safe manner, in accordance with standards of care.
- Collaborate with the RN to ensure the patient’s and caregiver’s goals and needs are being met.
- Structured Family Care providers must report changes in the patient’s condition the same day that a change is observed to an RN Case Manager or the Clinical Department.
- Provide electronic documentation of care provided during each day. Contact the Scheduling Department for any changes to Structured Family Care schedule.
- Address patient needs in a manner that supports and enables the individual to maximize abilities to function at the highest level of independence as possible.
- Preserve the dignity, self-respect, and privacy of the patient by ensuring high-quality care.
- Employees must be able to pass a fit-to-work screening including the ability to bend, lift 50 pounds, push, pull, walk, stand, climb stairs, full range of motion of all extremities, and be able to get up and down from the floor. Additionally, all employees under this job description will be free from communicable disease.
