Job Description
At Hi-Desert Medical Center (HDMC) we've been caring for our community for over 40 years. Our Continuing Care Center (CCC), located on the campus of HDMC, provides specialized care to residents close to their own communities, with spectacular panoramic views of desert landscapes for both residents and their guests
The Continuing Care Center has 120 beds and offers three distinct levels of service in semi-private rooms :
Subacute care : This is the highest level of care available in our long-term care setting. The CCC provides 25 beds dedicated to residents with ventilator and tracheostomy requirements. Clinical staff is specially trained to meet the unique needs of these patients. The CCC is one of the few facilities in Southern California providing this level of care.
Skilled nursing care : We provide long-term skilled nursing care to residents who require assistance with personal and medical needs. Care is provided to our patients who require rehabilitative therapies, wound care or recovery from accidents or invasive procedures.
Long-Term Care : We provide long-term care to residents who require assistance with personal and medical needs.
Position Summary
The Infection Control Preventionist coordinates a comprehensive infection prevention and control program in accordance with current requirements of regulatory, government, and accrediting / licensing agencies and organizational policies and procedures. Duties include surveillance for health care-associated infections (HAIs), collection, analysis, and interpretation of surveillance data, outbreak investigation, and program evaluation. The Director is also responsible for the day-to-day administration of the infection prevention and control program.

Responsibilities
Primary Duties
1. Develop and maintains an infection surveillance, prevention, and control program based on the organizational risk assessment and other needs in accordance with state, federal, and other reporting requirements.
2. Collects, manages, and analyze surveillance data using standardized methodology and defined criteria for HAIs.
3. Organizes and maintains surveillance data and records
4. Reports surveillance data, findings, and analysis to the appropriate committees, patient care units, personnel, and external agencies
5. Monitors patients for the occurrence of communicable diseases, HAIs, and resistant and epidemiologically important organisms
6. Monitors infectious diseases and epidemiologically significant organisms occurring in the community via laboratory reports and public health reports
7. Identifies and evaluates clusters of infections and potential outbreaks
8. Institutes an outbreak investigation and control measures, as needed
9. Recommends isolation precautions according to organization's policies and procedures
10. Develops infections prevention and control policies and procedures in accordance with requirements of regulatory, government, and accrediting / licensing agencies and recommendations of relevant regulations, standards, and evidence-based guidelines.
11. Update policies and procedures, as needed, to maintain compliance with regulations, standards, and evidence-based guidelines
12. Assists the organization to evaluate and maintain readiness for state and accrediting agency surveys
13. Monitor and evaluates the efficacy of infection preventions and control strategies
14. Collaborates with all departments of the facility to design and implement strategies to prevent HAIs and transmission of multi-drug resistant organisms and other epidemiologically important organisms
15. Investigates exposures to communicable disease and initiates appropriate measures to prevent the transmission of disease
16. Maintains confidentiality of patients, personnel, and related information
17. Provides consultation services related to infection prevention and control to the various departments in the District
18. Evaluates products, devices, and equipment as relating to infection prevention and control
19. Actively participates in performance improvement activities
20. Maintains departmental records to be in compliance with regulatory and accrediting agencies
21. Provides written and verbal reports to administration and relevant committees, departments, personnel, and external agencies
22. Communicates with local and state health departments
23. Participates in orientation programs and required annual education programs
24. Conducts in-service education on hand hygiene, aspetic technique, infection prevention and control strategies, antiseptics and disinfectants, specific infectious diseases, other related healthcare issues
25. Develops or obtains educational material and programs, as needed, for personnel, patients and visitors
26. Collaborates with employee health personnel on matters such as exposure management and immunization
27. Works with facilities management and other personnel to evaluate infection risks associated with renovation and construction projects and to identify control measures
