Position Summary :
Utilizes an interdisciplinary approach to organize, coordinate, monitor, evaluate and modify plans of care, focusing on complex medical and psychosocial needs of the Hospital's patients and their families. Performs pre-admission screening review of patients. Assesses and plans interdisciplinary plan of care that effectively manages patient along the continuum. Ensures appropriate use of resources and coordinates discharge planning.
Qualifications :
EDUCATIONAL REQUIREMENTS :
RN, licensed in Massachusetts
BSN or commensurate experience
Case Management Certification preferred
Computer literacy in Word, Excel, Outlook
EXPERIENCE REQUIRED :
5 years clinical experience Required
Minimum 1 year Case Management Experience Required
Case Management Certification preferred
Working Knowledge of InterQual Criteria preferred
Outcomes / resources management skills
Midas and MediTech Experience preferred
PROFESSIONAL LICENSURE AND CERTIFICATION REQUIRED :
RN, licensed in Massachusetts
BSN or commensurate experience
Case Management Certification preferred
Computer literacy in Word, Excel, Outlook
I NTERPERSONAL RELATIONSHIPS :
Communicates effectively with patients, families, visitors, and all members of the health care team.
PHYSICAL REQUIREMENTS :
Physical health sufficient to meet the ergonomic standards and demands of the position and its location.
