JOB TITLE: Case Manager
Program: CHAP (Comprehensive Housing Assistance Program)
Basic Functions:
To provide housing location or relocation support, budget counseling and case management to clients referred to the program. Build strong, effective professional working relationships with clients utilizing person-centered strategies. Demonstrate strong ethical values and maintain appropriate levels of confidentiality. This position is a contracted position with Penn Medicine/Lancaster General Health.
Essential Functions:
1. Provide direct case management, housing location, home visits and follow-up services to clients to ensure long term housing stability, prevention of homelessness, and further progress towards self-sufficiency.
2. Guide clients in developing and implementing a person-centered individualized housing plan including goals addressing income, budgeting and financial stability, overcoming existing barriers, securing permanent housing, and enhancing self-sufficiency to ensure long-term sustainability in housing to the fullest extent possible.
3. Maintain accurate client data in physical and electronic case files for each client
4. Accurately enter rent and utility payment data
5. Provide mediation and advocacy with landlords on client’s behalf to develop a workable plan to obtain and maintain housing.
6. Assist clients in identifying and utilizing available housing and utility assistance programs. Provide information and referral assistance regarding available support from appropriate social service agencies and/or community programs.
7. Apply knowledge of residential lease contracts to educate individuals of their rights and responsibilities.
8. Maintain client confidentiality and professionalism at all times
9. Transport clients as deemed necessary within Tenfold’s safety protocols. Transportation requirements should be limited to housing and job searches and occasional visit to relevant social service agencies.
10. Assist in the preparation of any required reporting as instructed by the Program Manager or Program Director
Minimum Knowledge, Skills and Abilities:
- Minimum Education Requirements- Bachelor's degree in social work or related field strongly preferred; Associate’s degree in social work or related field plus one year of relevant experience required.
- Ability to speak, read and write English and Spanish required
- Excellent communication skills especially listening, mediation and writing skills
- Strong organizational skills with ability to meet a demanding workload
- Detail oriented to complete requirements of files and contract compliance
- Adherence to the Housing First philosophy of ending homelessness.
- Ability to remain calm and interact professionally and effectively with clients, staff, service providers, and/or others in difficult, stressful, and/or crisis situations required.
- Sensitivity to the cultural and socioeconomic realities of those served.
- Commitment to the principle that everyone has strengths including the capacity to grow and change.
- An approach to service that is trauma-informed, recovery focused, and empowers people to resolve their own challenges to the fullest extent possible.
- Basic knowledge of tenant’s rights and responsibilities and case management best practices
- Concentrated record keeping, budgeting and mathematical skills
- Strong computer skills with proficiency in MS Word, Outlook, Excel, and data management systems essential.
- Demonstrated knowledge of county social service agencies and community programs or commitment to acquire knowledge
- Ability to work collaboratively with other personnel, service providers and/or other professionals.
- Capacity to maintain a helping role and to intervene appropriately to meet service goals.
- Current/valid driver’s license and a vehicle that is registered, inspected, and insured.
- Willingness to travel throughout Lancaster city and county and occasionally surrounding counties
