Restorative LPN

**Job Summary:**
We are seeking a Restorative Licensed Practical Nurse (LPN) to join our healthcare team. The Restorative LPN will be responsible for providing specialized care to residents to help maintain or improve their physical and psychosocial well-being. This role involves working closely with other healthcare professionals to create and implement individualized care plans.

**Qualifications:**
- Active LPN license in the state of practice.
- Proven experience as an LPN in a long-term care or rehabilitation setting.
- Knowledge of restorative care techniques and practices.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Compassionate and caring attitude towards patients/residents.
- Basic computer skills for documentation purposes.

**Responsibilities:**
- Assessing residents' needs and developing individualized restorative care plans.
- Implementing and monitoring restorative care interventions as prescribed by healthcare professionals.
- Collaborating with the interdisciplinary team to adjust care plans as needed.
- Providing hands-on care and assistance to residents with activities of daily living.
- Educating residents and families on restorative care techniques and goals.
- Documenting residents' progress and responses to restorative care interventions.
- Participating in quality improvement initiatives to enhance resident outcomes.
- Adhering to all state and federal regulations related to restorative care practices.
