About the Role
The ideal candidate will possess a Master's Degree in Social Work and current State Licensure in Social Work. A minimum of one year's experience in health care or hospice setting is also required. Your expertise will enable you to conduct psychosocial assessments, counsel patients and families, and provide accurate documentation of patient services.
Key Responsibilities
