Job Description
">We are seeking a highly skilled Clinical Care Coordinator to join our growing team. In this role, you will work closely with patients, families, and healthcare professionals to provide comprehensive psychosocial support and assessment.
The ideal candidate will possess a Master's Degree in Social Work (MSW) and current licensure as an LSW or LCSW. A minimum of one year's experience in healthcare or hospice settings is required, along with a valid driver's license and proof of insurance.
Responsibilities
">* Perform timely psychosocial assessments of patients and their families/caregivers.* Ensure identified needs are addressed in the patient's plan of care.* Provide exceptional service to patients, families, referral sources, co-workers, and other customers.* Counsel patients and their families/caregivers as needed regarding stress and coping difficulties.* Assist with applications for Supplemental Security Income, Medicaid, and Medicaid Waiver.* Liaise between patients and families/caregivers and community agencies.
