About the Role
This full-time position requires:
- A graduate degree from an accredited registered nursing program
- Licensure as a registered nurse in the state of practice
- CPR and First Aid certification

Responsibilities
The successful candidate will assume primary responsibility for patient and family caseload, including:
- Assessment and determining eligibility
- Admission processing, planning, implementation, and evaluation phases of the nursing process
- Obtaining data and assessing necessary information from patient physical, psychological, social, and spiritual factors
- Initiating intervention by other members of the interdisciplinary team
- Communicating with the attending physician and other agencies for optimal care coordination
- Providing regular visits to re-assess patient needs and update the plan of care
- Performing clinical pronouncement of death and providing support to the family
- Maintaining accurate patient records, charts, progress notes, reports, and preparing care plans timely
- Supervising and documenting delivery of care by the Home Health Aide as required
