About the Role:
This role requires a skilled Registered Nurse who can provide individualized care to patients, assess patient needs, and develop and implement effective care plans. You will collaborate with the interdisciplinary team to coordinate patient care and achieve optimal outcomes.
Responsibilities:
- Develop and implement individualized care plans based on patient assessment and needs.

- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
Requirements:
- Graduate from an accredited registered nursing program and licensed in the state.
- At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation, or medical surgical nursing (preferred).
- Home health experience preferred.
- Possess and maintain valid CPR certification.
- Must have reliable transportation, current driver's license, and appropriate automobile insurance.
