We are looking for a reliable LPN (Licensed Practical Nurse) to care for patients in a professional and considerate manner in their homes. You will work under the supervision of a registered nurse or doctor. LPN professionals must be first and foremost qualified and compassionate. The ideal candidate will be experienced in providing diverse basic care to those who need it while remaining patient and calm. It is a demanding job but also very rewarding.
Responsibilities
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care in clinical and progress notes.
- Take and record measurements of blood pressure, temperature, heart rate etc.
- Monitor patients’ condition including fluid intake and output and compose patient charts
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Collect samples for lab testing as assigned
- Provide emotional and psychological support when needed
- Communicate with patients’ family or friends to provide advice, comfort and release instructions
Skills
- Three (3) years nursing experience preferred. One or more years of Home Health experience is best.
- Understanding of confidentiality obligations and nursing best practices
- Excellent knowledge of medical and hospital terminology
- Good knowledge of MS Office and data entry
- Perfect physical condition and stamina
- Excellent communication and interpersonal skills
- Compassionate and able to handle stress
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
- Possesses and maintains CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
