Purpose of Job Position
Identifying, treating, following-up on patient wounds, providing treatments on a day to day basis in accordance with physician's orders, and documenting.

Responsibilities and Duties
Wound Care
- Perform treatments as directed by the physicians. Keep physicians informed of any changes in resident's skin conditions.
- Complete Decubitus and Non-Decubitus Skin Condition Reports Weekly, Monthly, and Quarterly. Attend and present Skin Condition Reports at the weekly SOC meeting
- Ensure all treatment orders written contain necessary information to provide clear instructions of how the treatments are to be performed.
- Make weekly rounds with Wound Specialty Physicians.
- Ensure Treatment Carts on each unit is stocked.
- Manage and order "Part B Medicare" wound supplies.
- Ensure Administration records are kept current.
- Ensure treatments are performed in a timely manner.
- Safety standards, written aseptic and isolation techniques are to be implemented and maintained at all times. Continue to implement and maintain programs for monitoring communicable and/or infectious diseases among residents and personnel and procedures for reporting hazardous conditions and/or
Restorative Duties and Responsibilities
Record all entries on flow sheets, notes, charts, electronic medical record etc., in an informative and descriptive manner.
Use only authorized abbreviations established by this facility when recording information.
Report all changes in the resident's condition to the Nurse Supervisor/Unit Manager or
Charge Nurse as soon as practical.
Report all accidents and incidents observed on the shift immediately.
Assist residents in preparing for activity and social programs (i.e., church services, patties, visitors, etc.).
Assist in transporting residents to/from appointments, activity and social programs, etc., as necessary.
Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
Assist residents to walk with or without self-help devices as instructed.
Change dressings, bandages, binders, etc., as instructed.
Assist with the application of non-sterile (moist and dry) warm/cold compresses
