JOB TITLE: Registered Nurse-MDS
DEPARTMENT: Nursing
REPORTS TO: Director of Nursing Services
The Registered Nurse-MDS (RN-MDS) collects and assesses, in accordance with State and Federal requirements, the information for the health and well-being of patients and residents from the time of admission through the end of their stay. The RN-MDS also helps to develop and implement a plan of care that meets the individual needs of each patient and/or resident.
- Provides direct nursing care within the scope of practice to patients and residents within the facility.
- Maintains a system by which new admissions, annual reviews, and quarterly reviews are to be initiated and completed within the required time periods.
- Conducts observations and interviews as well as evaluations required for MDS and/or care plan preparations.
- Completes the Minimum Data Set (MDS) within 7 days of admission in coordination with other members of the Inter-Disciplinary Team.
- Reviews orders with the appropriate nurse and/or clinician daily or more frequently if necessary.
- Reviews accidents/incidents and critically ill/unstable residents.
- Reviews New Admissions/Medicare Specialized therapies, i.e., IV Therapy and Pressure Sores, Stage 3 and 4.
- Reviews and assesses the condition of patient/resident within 24 hours of a reported accident/incident.
- Maintains appropriate documentation, charts, and related records.
- Reports significant observations to appropriate clinical staff including, but not limited to Charge Nurse, Director of Nursing Services, and Physicians.
- Actively participates in meaningful patient and resident rounding.
- Ensures the comfort of patients and residents.
- Other duties as assigned.
- Must be a graduate of an accredited School of Nursing.
- Must possess a current Registered Nurse license for the state of hire.
- Must possess a current MDS certification by the American Association of Nurse Assessment Coordinators (AANAC).
- Must possess a current Basic Life Support (BLS) certification.
- Must have the ability to read, write and follow oral and written directions at a level necessary to accomplish this job.
- Must have the ability to relate positively and favorably to residents and families and to work cooperatively with the other associates.
- Must maintain regular attendance.
- Must meet all local health regulations and pass post-employment physical exam, if required. This requirement also includes drug screening, criminal background investigation, and reference inquiry.
- Must be able to communicate verbally in a positive and professional manner.
- Must be capable of performing the essential job functions of this job, with or without reasonable accommodation.
Physical and Sensory requirements (with or without the aid of mechanical devices): Walking, reaching, bending, lifting, grasping, fine-hand coordination, pushing and pulling; ability to communicate with residents; ability to distinguish smells and temperatures; ability to read and write; and ability to remain calm under emergency situations and while dealing with multiple tasks.
