The Clinical Pharmacist's day is dynamic. The role involves reviewing and verifying medication orders, consulting with healthcare providers, and providing drug information to ensure safe and effective patient care. They conduct patient counseling, manage intravenous admixtures, assist with pharmacy systems, and handle prescription dispensing. The pharmacist also addresses patient inquiries, supports vaccine distribution and administration, and ensures inventory control, all while collaborating with the healthcare team to optimize therapy outcomes and patient safety. What does a typical day look like for a Pharmacist?

They conduct patient counseling, manage intravenous admixtures, assist with pharmacy systems, and handle prescription dispensing. The pharmacist also addresses patient inquiries, supports vaccine distribution and administration, and ensures inventory control, all while collaborating with the healthcare team to optimize therapy outcomes and patient safety. The Pharamcist will perform the following duties below:
- Filling pre-exchange and missing dose orders with the medication and delivery of the medication.
- Filling unit-dose carts as scheduled from the pick list and updated pick list reports including the operation of the unit-dose packaging equipment.
- Exchanging unit-dose carts as scheduled.
- Adjusting unit-dose drawers to reflect patient admissions, discharges, and transfers and clarifying patient locations, status, etc. with nursing and medical administration staff.
- Answering telephone calls.
- Preparing IV admixtures (piggybacks, large volume parenterals and chemotherapy) utilizing aseptic techniques in a timely manner for final check by a Clinical Pharmacist.
- Maintaining pre-determined stock levels.
- Utilizing the VistA computer system to maintain records and produce reports.
- Working closely with the Lead Technician or Supervisor to respond to changing use patterns and resolve problems.
- Verifying that a medication use evaluation (MUE) has been completed for all medications requiring a MUE as identified by the Pharmacy & Therapeutics (P&T) Committee.
- Verifying that a Non-Formulary Consult/request has been approved, prior to dispensing, for all medications/supplies identified as Non-Formulary by the P & T Committee.
- Reviewing and evaluating changes in therapy - questionable therapy and potential medication problems are discussed with prescribers and alternatives are recommended.
- Providing sound clinical information and make recommendations on which drugs are therapeutically equivalent, dosage differences, cost comparisons, etc. upon request by the clinical staff.
- Ensuring that the Patient Rx Status accurately corresponds to their eligibility.
- Prepare individual syringes for dosing vaccine, which includes: dilute vaccine, draw up doses from multi-dose vaccine vial; label finished product with all pertinent information (lot number/expiration date and time, etc.); affix tamper evident cap; store finished product (syringe) appropriately before administration; coordinate with medical staff regarding number of patients and when patients are scheduled to receive doses in order to minimize “waste”; and document and report pertinent/required information regarding vaccine preparation and usage daily.
- Other tasks as assigned.
- Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
You might be the person we’re looking for if you have:
- Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.).
- Or a graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
- Current state pharmacy licensure and the ability to practice as a pharmacist.
- Strong knowledge of drug therapy management, clinical pharmacy services, and regulations.
- Ability to work independently and as part of a healthcare team to optimize patient care outcomes.
- Excellent communication and interpersonal skills, with a focus on patient care and customer service.
- Working knowledge of outpatient automated dispensing systems, preferably ScriptPro.
Work Environment:
- This position may require working in various clinical settings, including inpatient and outpatient environments, depending on assigned tasks.
- May require interaction with patients, healthcare providers, and pharmacy staff.
