This individual provides direct care services (personal assistance and independent living skills training) and supervision of the patient in the own home. - Education / Experience :
- GED / High school diploma with a minimum of 1.5 years of behavioral health experience or,
- An associate's degree in the field of medical or behavioral health, or
- A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
- A bachelor's degree in the field of behavioral, or
- A master's degree or higher in the field of behavioral health
- Customer service skills, especially good communication skills are needed
- Ability to work independently, with flexibility, and within prescribed deadlines
- Basic computer skills
- Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health's driving policy.
- Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
