GENERAL RESPONSIBILITIES
Under the supervision and leadership of the Nursing leadership team, is responsible for providing patient care either in person or virtually as well as coordinating day-to-day nursing care and functions provided by nursing care team members in accordance with established standards of patient care and all Terry Reilly policies and procedures. Ensures support for clinicians, fosters teamwork and quality patient care, and implements and enforces protocols, policies and procedures in accordance with a Patient Centered Medical Home. Takes lead role in responding to crisis situations/critical patients.
ESSENTIAL DUTIES
- In accordance with Idaho State licensure and approved Terry Reilly standing orders, assesses and provides patient care responsive to patient needs. Provides education and supportive services consistent with the care provided in a comprehensive Patient Centered Medical Home care delivery system. Promotes an efficient, positive and safe environment for patients and care team members.
- Supports patient flow, including intervening in elevated patient care needs to ensure a safe efficient patient flow and utilization of care team resources. Continuously monitors patient and care team member needs and facilitates/assists as needed.
- Provides direct patient care either in person or virtually. Performs clinical interventions, patient education, generates all necessary EHR documentation and completes support duties in a timely, professional and efficient manner.
- Assists in screening walk-in and virtual patients, and arranges for them to be seen promptly, as per established protocols.
- Completes medical screening, medication administration, lab services, medication refill and in-basket support, and any other standing orders as available.
- Conduct home visits to support clinical care needs such as remote home monitoring or hospital follow up support where appropriate.
- Provides daily care coordination/registry management, and documents same, for assigned patient panel, per existing TR protocol by working with patients, families, providers and team members to support the implementation of individualized care plan and promote timely access to needed care.
- Assists with onboarding of clinical support staff members as needed. Assists the RN Supervisor with the maintenance of equipment, safety inspections, supply inventory, autoclave procedures and monitoring, emergency equipment checks and other procedures involved with supplies and equipment.
- Working with the high-risk patients registry and engaging high-risk patients to be healthier.
MINIMUM QUALIFICATIONS
- Current Idaho Paramedic license
- Current BLS certification.
- 1-year paramedic experience.
- Strong problem solving, time management and computer skills.
- Ability to communicate effectively and in a professional manner with all care team members, supervisors, and patients.
- Ability to establish and maintain a positive, efficient and safe working environment.
- Able to walk and stand for long periods of time.
PREFERRED QUALIFICATIONS
- 2 years of clinical experience in a physical or virtual clinical care environments.
- Bachelor's degree in health science or related degree.
- Experience in a community health clinic environment or community paramedic setting working with a wide variety of patient populations, including patients living in poverty and individuals of minority groups.
- Bilingual abilities (English/Spanish).
- Ability to lift 50 pounds.
