Care Coordinator works with the targeted population by providing community-based outreach and advocacy for individuals with mental health, substance use, and healthcare difficulties. They educate clients about their condition, connect them with health and behavioral health providers, and evaluate their progress. They work as part of an inter-disciplinary care team.
Physical Demands:
1. While performing the duties of this job, the employee regularly operates a computer and other office machinery.
2. Frequently works outdoors and is exposed to outdoor weather conditions.

Duties and Responsibilities
1. Facilitates relations between the agency and the community by communicating agency policies and programs to clients, family members, and community residents and addressing stigmas related to services.
2. Facilitating linkages to address client needs and
3. Ensuring continuity of care among providers.
4. Conveying community cultural patterns and attitudes to agency’s professional staff.
5. Conducts community outreach and attends community-based events.
6. Receives referrals from individuals, social workers, and other referral sources to assist recipients in acquiring needed services.
7. Serves as an advocate and assists clients with accessing agency and community resources.
8. Other duties as designated.
Qualifications
Position Qualifications/ Requirements:
1. A bachelor's degree in social work, or health care administration or a related field.
2. Two years case management experience providing direct services to individuals with severe mental illness, and/or individuals experiencing substance misuse.
3. Commitment to the mission of the agency and care coordination.
4. Strong understanding of cultural competency with the targeted population.
5. Good communication and interpersonal skills and ability to speak concisely to clients.
6. Ability to interact with Care Team members.
7. Organized with confidential client material, appointment tracking, and caseloads.
8. Ability to build relationships with different types of people, including clients, organization members, and members of the health care team
9. Possession of valid Class C or higher driver’s license and insurability by the agency insurance carrier(s). Basic computer skills.
