Description
A Parent Partner in the Wraparound Division will work closely with the Wraparound child’s parent/caregiver in order to represent their best interests and shall participate as a member of the Child and Family Team (CFT). For children placed in out-of- home placement, the Parent Partner will work with both the out-of-home caregiver and parent/ current caregiver.
Scope of Position:
1. Participates in Wraparound CFT Matrix meeting using the 4-Step Model and adheres to Wraparound Policies and Procedures.
2. Maintains active communication with treatment team, DCFS, Probation, and DMH in adherence with the Shared Core Practice Model.
3. Participates in development of safety plan with Treatment team and family in order to ensure client safety.
4. Provides in-home and in-community services to families, formal, and informal supports to team, assess, track, adapt, and plan to meet the needs of the client/family.
5. Facilitates weekly Parent Support Groups.
6. Participates in multi-disciplinary team in developing effective strategies to support and ensure client/family success.
7. Provides direct linkages to community resources and local systems to ensure needs to client/family are being met.
8. Other duties as designated by supervisor, manager and/or program head.
Qualifications
A Parent Partner in the Wraparound Division is, or has been, the parent or primary caregiver (not foster parent) of a child who received services from one of the following COUNTY Departments: DMH, Probation Department, and/or DCFS; or from Regional Center. A Parent Partner must, at the time of hire, no longer have an open case in either the Probation Department or DCFS.
At the time of hire the Parent Partner must not have an open probation case for at least one (1) year. For DCFS cases, if closed for for less than one (1) year, written approval is required from DCFS Program Manager. Request will be reviewed and responses will be provided on a case by case basis.
