What are the responsibilities and job description for the RN Navigator Oncology position at Teton County Hospital District?
JOB SUMMARY
RN will coordinate patient care throughout the continuum in collaboration with the multi-disciplinary team. RN will
serve as a clinical resource with expertise in hematology/oncology care management. RN will assist Oncologist and
the Oncology Advanced Practice Nurse in the day-to-day care of patients. Involves direct patient interactions by
phone and assisting with office visits. Responsibilities include: clinic visit intake process, patient and family education,
assistance with management of cancer-related symptoms, medication administration, venipuncture, overseeing the
oral chemotherapy program, tracking if survivorship care-plans and assisting with follow-up surveillance. Delivers
services according to the Scope of Service of OPM. Adheres to and practices the SJH Corporate Compliance
Program and participates in Performance Improvement activities.
ESSENTIAL FUNCTIONS
Patient Care
Understands the side effects of chemotherapy, biotherapy, and immunotherapy. Assess patient and assists with
general symptom management. Document patient concerns, patient messages, and care provided to the patient in
the patients medical record for the provider to review. Monitor and recognize changes in the physical and
psychological status and tales appropriate action based on assessment of the patient and family. Performs triage of
patients over the phone to determine and prioritize level of care needed. Work with the provider daily to manage
acute symptoms management calls and the scheduling of add-on clinic visits.
Assists Oncologist and Advanced Practice Nurse with clinics by checking patients in, updating mediations and other
pertinent medical history, drawing labs, giving injections, and providing on-going education and support. Assist
providers with survivorship care=plans and follow-up surveillance.
Practices autonomously and consistently with professional nursing standards.
Organizes and prioritizes care of multiple patients with varying levels of acuity in a timely and efficient manner with a
positive attitude.
Manages the oral chemotherapy program. Works with providers to monitor labs, provide patient education, create
and update adherence calendars and monitor symptom management.
Patient and Family Education
Plan and provide education on treatment modalities, chemotherapy, symptom management and follow-up care to
patient and family based on physical, behavioral, psychological, developmental, and age-related factors.
Assess patient and family needs in regards to: physical, emotional and social aspects of the disease and/or
treatment. Assists in the development and implementation of an individualized care plan and modify plan of care as
needed.
Initiates and incorporates patient and family education onto the plan of care and nursing practice.
Identifies continuity of care needs for discharge and follow-up using appropriate resources.
Communicates up-dated status reports and clear information to patient and family to make informed decisions.
Unit Specific Duties
Functions in an organized and time conscious manner. The RN partners with patients, families, the interdisciplinary
team, and community resources to provide well-coordinated, timely compassionate, exemplary, interdisciplinary care.
The RN communicates with all members of the healthcare team, as appropriate, about patient/family needs and
concerns.
Serves as a resource for community educational events such as health fairs, screenings, and symposiums. Provides
community outreach and program support including facilitating cancer support groups.
Training
Will attend occasional hands-on trainings at the Huntsman Cancer Center in order to stay up to date with best
practices in oncology and keep SJH aligned as a Huntsman affiliate.
Minimum Education
Required:ASN and completion of the ONS chemotherapy administration course within 6 weeks of hire.
Preferred:BSN, Oncology Certified Nurse (OCN)
Required:Minimum of three years nursing experience in acute care/oncology nursing. Must be able to communicate
professionally and effectively. Excellent interpersonal and customer service and phone skills. Basic computer skills.
Required Licenses:Current RN License with authority to practice in the State of Wyoming. Current BLS at time of hire and ACLS required
within 6 months of hire.
FUNCTIONAL DEMANDS
Working Conditions
Hospital based outpatient oncology department, occasional stressful situations. Occasional need to provide care in
inpatient departments. Must be on-site at the SJH campus or other facilities to fulfill the responsibilities of this role.
Physical Requirements
Adequate sight/hearing and other senses to perform job requirements. Ability to lift 50 pounds. Ability to push, pull,
bend, reach. Intermittent standing, walking, kneeling, sitting, computer and phone use.
