Intellectual Developmental Disability (IDD) Habilitation Coordinator – Enhanced Community Coordination (ECC) and Waiver
Role Responsibilities
The Habilitation Coordinator manages a blended caseload of Service Coordination for individuals enrolled in the HCS waiver program and Enhanced Community Coordination to individuals transitioning or diverting from institutional care. The Habilitation Coordinator is responsible for the following:
- Assessment, service planning, monitoring, advocacy and crisis prevention and management for assigned caseload of individuals with IDD.
- Provision of Medicaid billable targeted case management activities for a caseload of up to 30 individuals enrolled in the HCS waiver program or experiencing a transition in care.
- Compliance with applicable local, state and federal rules and regulations.
- Adherence to and promotion of the Texana Compliance Policy.
- Travel within 6 county region.
- Work hours: M-F, 8-5 with flexibility based on client need.
Salary
- $21.69 per hour - $25.25 per hour based on years of experience
Required Education and/or Certifications, Experience, Skills
- Bachelor degree in a social, behavioral, human services field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education and criminal justice.
- Texas Driver License (in good standing)
- Two years of experience as Service Coordinator serving individuals with complex needs required.
Preferred Education and/or Certifications, Experience, Skills
- Strong organizational and computer skills (Microsoft Office and Electronic Health Record system).
- Knowledge of State Rules relevant to Habilitation Coordination, Service Coordination and PASRR.
- Experience working with individuals with IDD with complex medical and/or behavioral health needs.
Job related Physical Requirements
- Must be able to perform SAMA and CPR. Training is provided to employees once hired
- Must have ability to see, write, operate office equipment, operate a motor vehicle and have depth perception.
- Qualified individual must be able to perform these functions with or without reasonable accommodations
