Job Summary

Responsibilities
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide counseling and support services to clients, including crisis intervention when necessary

- Advocate for clients' rights and access to resources within the community
- Maintain accurate and up-to-date client records and documentation
- Monitor and evaluate clients' progress towards their goals
