Brief Job Description
Under the supervision of the Case Coordination Supervisor in the Blood Lead Surveillance Branch (BLSB), this position performs highly complex (senior-level) public health work and serves as a Follow-up Coordinator responsible for provider and parent notifications for children with elevated blood lead levels. Work involves the coordination and planning, and monitoring of the Childhood Lead Poisoning Prevention Program (CLPPP) follow-up activities of children with elevated blood lead levels, within operational parameters. Provides consultation and education to providers, and to the public about issues related to the follow-up of children with lead poisoning. Works under minimal supervision, with considerable latitude for the use of initiative and independent judgment.
Essential Job Functions
EJF 1: Coordinates, plans, and monitors case coordination activities for children statewide who have blood lead levels at or above the reference value; and in coordination with the Case Coordination Supervisor, develops goals and objectives for the Childhood Lead Poisoning Prevention Program (CLPPP). Case coordination activities include but is not limited to the following: Processing notifications to healthcare providers and parents statewide on blood lead results and providing health education on lead poisoning prevention and sending notifications timely and correctly; Providing consultation and communication with healthcare providers statewide via phone, letter, fax, and e-mail on a daily/weekly basis about the follow up and care of children with elevated blood lead levels, including immediate contact with providers for children with critical blood lead levels; Documenting all actions in the appropriate notification system, including the status of child, communication activities, and other actions as appropriate while maintaining confidentiality of protected health information; and Providing oversight and operation of the notification system and evaluating the notification system need for changes, additional specifications, and updates. (45%)
EJF 2: Analyzes data for planning and coordinating the Childhood Lead Poisoning Prevention Program (CLPPP) case coordination activities. Communicates extensively with Case Coordination team members on a daily/weekly basis to assist with issues related to the follow-up of children with elevated blood lead levels and related situations. Makes recommendations and referrals as indicated by child’s status. Evaluates need for follow up activities for children with blood lead levels at or above the reference value. Monitors and identifies data discrepancies and makes suggestions for improvement. Evaluates system need for changes, additional specifications, and updates. (25%)
EJF 3: Assists in collecting, organizing, analyzing, and preparing materials in response to public health information requests. Receives public inquiries regarding blood lead and addresses them appropriately. Reviews incoming electronic and/or hard copy blood lead data reports, uses critical thinking skills and process documents to determine what action is needed, and distributes the information. Develops and implements updates in documentation and distributes as appropriate (15%).
EJF 4: Evaluates the effectiveness of training and education for Childhood Lead Poisoning Prevention education programs. Participates in development, documentation and review of policies and procedures based on data and program evaluation. Provides technical assistance and obtains missing information for blood lead data. Provides active outreach to obtain necessary information and trains healthcare providers on proper reporting and reporting laws related to blood lead data and on the education of childhood lead poisoning prevention. (10%)
EJF 5: Performs related work as assigned. “Other duties as assigned include but not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participations may require an alternate shift pattern assignment and/or location”. (5%)
