Job Summary
Provide case management and work with other members of the treatment team to develop and implement programs designed to meet the physical, developmental, and emotional needs of children in care and clients in our aftercare program.
Responsibilities
- Identify the special needs of children and aftercare clients and make referrals to appropriate resources as they relate to those needs.
- Arrange and conduct support groups for post-adopt clients.
- Conduct TBRI and other trainings, as needed.
- Work with adults who have aged out of foster care to make sure their needs are met.
- Develop and present individualized service and/or treatment plans to articulate goals and objectives for meeting the needs of children in care in accordance with licensing and level of care standards.
- Ensure thorough implementation of individualized service and/or treatment plans by all staff intervening on behalf of children in care.
- Assist children and adults with special needs as they relate to school, employment, or social enrichment activities.
- Provide special support services to children, adults and their workers.
- Work with the treatment team, as well as support personnel, to identify and meet the needs of children and adults within the after care and post-adopt program.
- Maintain current and complete case records and documentation of services and progress of children in care.
- Demonstrate cultural sensitivity to clients of any ethnicity, race, or socio-economic group.
- Perform duty as on-call staff.
Qualifications
- Bachelor's degree in social work or other related human services field.
- Experience in child care related areas or community resources, either through employment experience, volunteerism, or practicum placements while in undergraduate program.
- Prefer 2-3 years full-time experience in residential childcare or community resources.
- Be available outside of normal 8am-5pm hours and on call.
Job Type: Full-time
Pay: From $39,000.00 per year
