Position Overview:
The Advanced Practice Provider will provide comprehensive care, manage acute and chronic conditions, and support the clinics quality and operational goals. This role is perfect for a passionate clinician eager to collaborate with a multidisciplinary team and contribute to a culture of excellence.
Key Responsibilities:
- Conduct patient assessments, diagnose conditions, and develop treatment plans.
- Provide preventive care services, including screenings, immunizations, and counseling.
- Collaborate with physicians, nurses, and staff to ensure coordinated care.
- Contribute to quality performance initiatives, including HEDIS and MIPS metrics.
- Maintain accurate documentation in the EHR and ensure compliance with regulations.
Qualifications:
- Masters degree in Nursing (MSN) or graduate of an accredited PA program.
- Current, unrestricted license and certification (AANP, ANCC, or NCCPA).
- BLS and ACLS certifications (required).
- 13 years of primary care experience (preferred).
- Strong clinical, diagnostic, and teamwork skills.
