Advanced Practice Provider - Position Overview

The Advanced Practice Provider (APP) will collaborate with patients, their families, caregivers, primary care physicians, and specialists to deliver episodic acute care and chronic disease management for patients with progressive illnesses. This role involves making home visits to provide comprehensive care, including but not limited to symptom management, care coordination, and advanced care planning. The APP will work closely with an interdisciplinary team to ensure patients receive personalized, high-quality care with the goal of improving patient quality of life and reducing avoidable healthcare utilization.

Key Responsibilities :

  • Conduct home visits to assess, diagnose, and manage the healthcare needs of patients with complex medical and social conditions.
  • Provide supportive care to patients with advanced or chronic conditions, focusing on symptom management, comfort, and quality of life.
  • Develop, implement, and monitor personalized care plans in collaboration with the primary care physician and other members of the healthcare team.
  • Coordinate care with the patient's community providers, including specialists, home health agencies, social workers, community health workers and other support services.
  • Educate patients and families about disease management, treatment options, symptom control, and self-care techniques.
  • Monitor and manage complex medications, including pain management and other symptom-related treatments.
  • Assess the physical, emotional, and social needs of patients and develop appropriate interventions to address those needs.
  • Document all patient interactions, assessments, treatment plans, and follow-up care in the electronic health record (EHR).
  • Respond promptly to patient needs, providing timely follow-up and updates to the primary care physician group.
  • Participate in case conferences, interdisciplinary team meetings, and other collaborative activities to ensure holistic patient care.

Required Qualifications :

  • Current, unrestricted Nurse Practitioner or Physician Assistant license in Texas.
  • Minimum of 5 years of experience as an APP preferably with experience in home health, palliative care, or working with complex patient populations.
  • Valid Texas driver’s license
  • Knowledge, Skills, Abilities, and Competencies :

  • Strong knowledge of palliative care principles, symptom management, and care coordination.
  • Experience in home-based care or managing patients with chronic conditions.
  • Expertise in collaborating with the patient's primary care provider to ensure coordinated, patient-centered care.
  • Compassionate approach to care, focused on improving the quality of life for patients with complex needs.
  • Maintain a broad base of technical knowledge and skills to perform clinical and administrative duties.
  • Proficient in using software programs and electronic health record (EHR) systems.
  • Demonstrate effective teaching skills to communicate medical information to patients, families, and the interdisciplinary team.
  • Utilize excellent communication, interviewing, and counseling skills to explain medical issues and treatments in understandable terms.
  • Possess the ability to make clinical decisions in a home-based setting.
  • Prioritize tasks, and manage time effectively.
  • Ability to engage in moderate local travel (up to 90%).
