Job Summary: The Inpatient Care Coordination Specialist will be dedicated to superior customer service and satisfaction. This position will develop and maintain positive relationships with both internal and external customers while conveying a positive image. Coordinates and directs patient care to ensure patients’ needs are met and hospital policy is followed.
You Will Be Responsible For:
- Schedules physician follow-up appointments for patients discharged home
- Coordinates appointments for patients who do not have a documented primary care physician
- Coordinates appointments with Specialist if the physician dictates that follow-up is prescribed
- Contributes to the day-to-day processes of discharging patients.
- Three years of hospital experience
- Effective communication
- Effective follow-through
- Decision-making skills
- Independent judgment
