What You Will Do:
• PLAN of CARE, TECHNICAL SKILLS:
• Collaborates with team to facilitate hourly rounding and initiates action to meet patient and/or significant others need for information.
• Takes action to reduce/eliminate patient risks.
• Collects data and reports significant findings to RN.
• Safely administers medication in accordance with medication administration guidelines.
• Uses nursing model to obtain adequate data to initiate, implement, and revise plan of care to meet patient, physician, and family needs.
• Individualizes care measures to reflect specific patient population. Performs technical procedures safely, proficie ntly.
• Demonstrates critical thinking.
• Adheres to national safety goals in care delivery.
• Monitors patients.
• PROFESSIONAL ACCOUNTABILITY:
• Identifies realistic and measurable annual goals for professional nursing development.
• Serves as patient advocate.
• Complies with personnel policies, i.e. attendance policies, ID badge, dress code, LVN Scope of Practice.
• Communicates per department expectations.
• Recognizes and promptly communicates ethical and legal concerns through established channels of communicati on.
• Takes responsibility in continuing to develop own professional nursing knowledge and skills.
• PARTNERSHIP:
• Works in partnership with peers, other disciplines, and unlicensed assistive personnel.
• Actively participates in formal and informal teaching and evaluation to strengthen clinical competence of self and team members.
• Uses Unit Based Council structure.
• Flexible with scheduling to help meet departmental/patient needs.
• Adapts to changes in workload assignment to meet changing needs of the individual patient or the unit.

• Assists in the identification of ineffective and costly processes and takes action to improve those processes.
• In collaboration with RN incorporates evidence-based practice and research to monitor and continuously impact outcomes.
• Actively participates in unit-based programs.
