We are seeking a dedicated and compassionate Social Worker to join our team. You will play a vital role in advocating for clients, developing treatment plans, and facilitating group therapy sessions.
Responsibilities
- Conduct thorough patient assessments to identify individual needs and develop appropriate care plans.
- Maintain accurate medical documentation and ensure compliance with regulatory standards.
- Provide psychotherapy services to clients dealing with behavioral health issues.
- Facilitate group therapy sessions to promote social skills and peer support among clients.
- Collaborate with multidisciplinary teams within clinics and nursing homes to ensure comprehensive care.
- Utilize motivational interviewing techniques to empower clients in their treatment journey.
- Advocate for individuals with developmental disabilities, ensuring they receive necessary services and support.
- Monitor client progress and adjust treatment plans as needed based on ongoing assessments.
Experience
- A degree in Social Work or a related field is required; licensure is preferred.
- Experience in patient assessment and medical documentation within clinical settings.
- Familiarity with working with individuals with disabilities, both developmental and physical.
- Knowledge of behavioral health practices, including psychotherapy and group therapy techniques.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent organizational skills and attention to detail in documentation and reporting.
