Texas Hospice Providers is searching for an RN clinical liaison to network with local healthcare professionals and organizations as well as with individuals in the community. We are a small organization who genuinely cares about our patients and families and our goal is to ensure each of our patients are able to live out the remainder of their lives in peace, comfort, and security. Our RN liaisons are special because they are able to meet the patient and family at the point of their needs and address them in a way that non-nursing liaisons cannot. We are patient advocates first and foremost and our team is very collaborative. Our leadership truly cares about our staff and believes that if they take care of their team, the team will take care of the patients and families. Your Responsibilities:

- Develop and implement community outreach strategies to engage with target audiences

- Build and maintain relationships with community organizations, businesses, and influencers

- Collaborate with administrative leadership to identify and pursue leads for referral sources to promote census growth

- Develop care territories and recruit skilled RN/LVN/CNA talent to manage patients within each territory. Provide education on hospice care to referral sources, patients, families, and the general community.

- Collaborate with internal teams to create and execute marketing campaigns

- Manage social media platforms and develop content calendars to drive engagement and increase brand awareness

- Meet business objectives by forecasting growth requirements, and by maintaining and expanding client base pipelines through frequent and repeated rapport with key clients.

- Assess and admit eligible patients to the hospice census in conjunction with the interdisciplinary team

- Collaborate with nursing leadership to Identify patient and family needs prior to handoff to primary care team

- Maintain compliance with CHAP and CMS standards. Practice within the scope of the Texas Nurse Practice Act.

Skills:

  • Current RN License with experience in hospice care
  • Strong organizational and communication skills
  • Proven marketing skills, including digital and social media marketing
  • Familiarity with performance marketing and content creation
  • Ability to analyze care plans and patient needs
  • Ability to build mutually beneficial personal relationships that contribute to team goals and organizational mission
  • Valid Drivers License in good standing and reliable transportation
  • Clinical knowledge to assess patients to determine hospice eligibility and advocate for their needs with the interdisciplinary group

Education and Advocacy: Educate patients, families, and the community about end-of-life care options and promote awareness of hospice services. Advocate for policies and practices that support compassionate end-of-life care.

Palliative Care Integration: Collaborate with other healthcare providers to integrate palliative care principles earlier in the disease trajectory, ensuring that patients receive appropriate support from the time of diagnosis through the end of life.

Continuous Improvement: Regularly assess and improve the quality of care provided. This involves ongoing evaluation of processes, staff training, and incorporating feedback from patients and their families.

Ethical and Legal Compliance: Adhere to ethical standards and legal regulations governing end-of-life care. Ensure that all care provided aligns with the patient's wishes, respecting advance directives and end-of-life care plans.

Community Engagement: Actively engage with the local community to promote awareness, understanding, and access to hospice services. Build partnerships with healthcare organizations, community groups, and other stakeholders to enhance the network of support for patients and families.

Staff Well-being: Prioritize the well-being and professional development of the hospice care team. A supportive work environment contributes to the delivery of high-quality, compassionate care.

Job Type: Full-time

Pay: $65,000.00 - $75,000.00 per year
