Overview
We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. Responsibilities
- Conduct thorough patient assessments to identify social, emotional, and environmental factors affecting health and well-being.
- Provide counseling services for individuals dealing with mental health issues, addiction, or family dynamics.
- Develop and implement care plans tailored to the unique needs of patients, including those with autism or requiring hospice care.
- Collaborate with healthcare providers to ensure continuity of care and effective communication among all parties involved.
- Facilitate access to community resources and support services for patients and families.
- Offer guidance in navigating insurance benefits and financial assistance programs.
- Participate in case management meetings to discuss patient progress and adjust care plans as needed.
- Maintain accurate documentation of patient interactions, assessments, and interventions in compliance with regulatory standards.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited institution.
- Valid state licensure as a social worker (LCSW or equivalent).
- Experience in clinical counseling, mental health counseling, or related fields is preferred.
- Familiarity with addiction counseling techniques and resources.
- Knowledge of applied behavior analysis principles is a plus.
- Experience working with children and families in various settings, including home community care.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent communication skills, both verbal and written.
- Ability to work collaboratively within a multidisciplinary team environment.
