Social Worker (BSW) - ELP
Position Description
Under the supervision of a master prepared social worker, provides counseling to families, individuals and groups.
Works in collaboration with multidisciplinary teams, coordinates patient discharge planning, complies with required
standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff.

Major/Essential Functions
Investigate, assess and plan interventions to help patients cope with social, emotional, economic and environmental problems.
Interview patients, assess priorities and document case activity.
Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.
Assist clients/families with obtaining community assistance by referral to proper resources.
Develop and maintain working relationships with community health, welfare and social agencies and seek creative means to assist the client with his/her needs.
Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.
Adhere to all policies, procedures and practices (Regents Rules, TTUS, HSECEP OPs, Community Living Assistance Support Services Program/HHSC, etc.).
Personally demonstrate, display and act in accordance with TTUHSC EP's Values (Service, Respect, Accountability, Integrity, Advancement, and Teamwork). Serve as a Value's leader while actively promoting and encouraging staff across the institution.
Perform all other duties as assigned
Required Qualifications
Bachelors degree in social work from an accredited college or university.
Must be licensed by the Texas State Board of Social Work Examiners within 6 months of start date.
Preferred Qualifications
Ability to provide counseling to individuals, families, and groups; psychosocial evaluation; advocacy; and referral to community resources.
Effective human relation skills.
Ability to communicate effectively in both verbal and written.
Minimum 6 months of social work experience.
Working knowledge of community agencies and services available for the special needs of assigned patient population.
Bilingual in Spanish/English is desired.
