What are the responsibilities and job description for the CLASS Service Coordinator position at Texas Visiting Nurse Service?
This job requires that home visits be made on a monthly basis to individuals who are in the Community Living Assistance and Support Services (CLASS) Program, which is a Medicaid waiver program through Health and Human Services Commission. The CLASS Service Coordinator must conduct visits on a monthly basis to individuals in this program visits consist of Supervisory visits, Special Planning Team visits, Evaluations and Trainings on attendants that are employed through CLASS, conduct new hires of attendants, follow-up on complaints and/or incidents and complete other in office duties.
Must be willing to travel, areas of coverage are from the Lower Valley and Mid-Valley, and as needed the Upper Valley.
Must be willing to take on-call.
Must be CPR Certified
Bilingual is preferred (English and Spanish)
Must have good Communication Skills and be computer literate.
Qualifications:
Must have a High School Diploma and/or GED with 4 years work experience in the delivery of services and supports to persons with related conditions or similar disabilities; or have a bachelor's degree in a health and human services field and two years' work experience in the delivery of services and supports to persons with related conditions or similar disabilities; or
A Licensed Vocational Nurse
