We are seeking a Registered Nurse - Home Health Full Time & Part Time/ Per Diem to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients in a home health setup. Let us know which areas you can service and we'll match you with the right referrals!
Responsibilities
- Administration of prescribed medication, treatments, and therapies.
- Patient assessments and coordination of care Health, promotion,
- Providing nurse visits to home health patients. (home visits)
- Administer nursing care to ill, injured, or disabled patients
- Diagnose and establish patient treatment plans
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating physicians or specialists regarding patient care
- Educate patients about health maintenance and disease prevention
Qualifications
- Previous experience in nursing or home health
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
Requirements
- Professional License
- Academic Diploma
- Updated CV/Resume with Experience
- Driver's License
- Auto Insurance
- Active BLS or CPR Certification
- Recent Physical Examination OR Fit-to-Work Note from Physician
- Valid (1 year) TB/PPD Test OR Valid (3 years) Chest Xray
- Covid Vaccine Card/Records (Includes Booster/s)
- Influenza Vaccine (or Declination)
- MMR Immunization Records
- Hepatitis B Immunization Records
- Professional Liability Insurance
- Photo (For Issuing of L&G Nursing Group ID)
