Position Overview:
Case Management is a collaborative process of assessment, planning, facilitation, coordination, evaluation, outreach and advocacy for options and services to meet an individual’s and family’s comprehensive needs through communication and available resources to provide equitable access to all students.
We are seeking a skilled and motivated Case Manager to join our team. The ideal candidate will have a combination of exceptional skills in human/social services, problem-solving and communication, as well as a focus on student development and support. The Case Manager will play a pivotal role in managing and resolving student relations in and outside of the training program, creating a positive and inclusive learning environment and managing student relations by facilitating conflict resolution within the training program.
Case Manager Departments:
Recruitment and Placement- Interviewing of potential participants for the program. This is ongoing and performed on a monthly basis during the second week of each month. To be complete by the end of the month proceeding the new term.
Barrier Reduction- Specializes in providing resources and opportunities to help mitigate any barrier creating a hinderance to completing the program.
Student Support- Provides daily support and assistance to participants. Performs weekly check in's. Communicates progress and/or barriers to the necessary personnel.
Responsibilities:
· Provides proactive, home based, face to face case management.
· Conducting student intake interviews, assessing needs, and formulating individual employment plans.
· Establishing and maintaining rapport with students to enhance the overall student experience and well-being.
· Tracking and managing students from intake to testing, facilitating career development, and ensuring successful job placement.
· Implementing barrier reduction strategies to support students in overcoming obstacles.
· Proactively engaging students through face-to-face case management, ensuring a personalized approach.
· Collaborating with various stakeholders to provide necessary resources and services.
· Collaborating with recruiters, trainers, program administrators, industry managers etc., to ensure consistent, effective communication and resolution of student issues.
· Developing, coordinating, and securing Federal, State, and local benefits for students.
· Creating and maintaining records, including case notes, events, and progress documentation.
· Facilitating weekly interdisciplinary meetings to communicate progress and address obstacles.
· Assisting students in obtaining key documents, transportation, and necessary resources.
· Teaching essential life skills, including financial skills and budgeting.
· Advocating for students to ensure fair and consistent services and public benefits.
· Establishing a working relationship with relevant providers and maintaining knowledge of community resources.
· Contributing to the development and implementation of a progressive engagement model.
· Document student progress, barriers, and IEP in the Case Management System.
· Developing and implementing policies and procedures to ensure that the training organization remains in compliance with employment laws and regulations.
· Staying up to date with local state education and employment laws.
· Other duties as assigned by the Executive Director or Administrative Director.
· Conduct intake interviews and assessments as needed.
· Collaborate with other departments and community organizations to ensure students have access to necessary services.
· Maintain constant communication between funding sources, case management team, and senior management.
· Maintain accurate and up-to-date records of student information, services provided, and outcomes achieved.
· Participate in staff training and development
Qualifications:
· Bachelor’s degree in human resources, Social Work, or related field.
· Proven experience in case management, preferably in a training or education setting.
· Strong interpersonal and communication skills.
· Knowledge of community resources and services available in the Illinois and specifically Chicago communities.
· Ability to engage and build rapport with diverse individuals.
· Detail-oriented with excellent organizational and documentation skills.
Job Type: Full-time
Pay: $85,000.00 per year
