AACI is a dynamic community health center dedicated to serving the underserved and marginalized populations of Santa Clara County. We believe in integrated behavioral health care, where medical conditions and related behavioral health factors are addressed together in one comprehensive setting.As an Integrated Behavioral Health Counselor, you will play a crucial role in our Health Center by assessing and treating patients, bridging the gap between physical and behavioral health. Your responsibilities will include : Providing culturally sensitive short-term therapy to referred patients.Collaborating closely with Health Center providers.Integrating care to enhance each patient’s overall well-being.Salary : $73,000.00 – $120,000.00 (Depending on experience, licensure, and education)What We Offer You : Fully Supported : Integrated team including case manager!No travel required : Outpatient in office and via zoom.Flexibility : Hybrid remote work (2 days Remote / 3 days onsite) with a company laptop.
