HEALTH HOME CARE MANAGEMENT TEAM LEAD
EDUCATIONAL REQUIREMENTS:
MSW or Master’s degree preferred, or Bachelors considered with three years of full-time work experience in human services.
Demonstrated ability and appreciation of administrative operations of DOH requirements for health home care management services
DUTIES:
Under the supervision of the Program Director:
SKILLS
· Excellent problem-solving skills
· Ability to communicate effectively

· Ability to assign and delegate tasks
· Good organizational skills
· Ability to work independently
· Expert with computer programs (e.g. Word, Excel)
· The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs.
· Ability to legally operate a vehicle in NY State per ACBC’s insurance carrier
MAJOR FUNCTIONS:
· Understand and implement all agency and program policies and procedures
· Assist program director as necessary and requested
· Carry a caseload when required
· Maintain compliance with all applicable regulations
· Monitor, evaluate and provide direction to staff following protocols
· Meet weekly with supervisee’s or more as necessary
· Provide guidance to supervisee’s
· Participate in rotating after hours/weekend on-call coverage
· Monitor staff training and performance issues, keeping program apprised
· Establish and monitor relationships with persons receiving services; assure individual needs are met
· Provide oversight/referral/coordination of clinical services
· Represent agency and program in the community and with other service providers
· Monitor and advocate for individual rights as needed
· Participate in safety training and development activities
· Participate in meetings as requested by program director
· Participate in all trainings as set forth by the NYS DOH program requirements
· Report and document incidents per regulations
· Understand and follow all program procedures
· Participate in and complete all required staff training and development activities
· Coordinate services with other community providers as needed
· Regular and substantial contact with individuals receiving services
· Perform additional duties, as assigned
