Providing compassionate care and medical management of patients experiencing severe medical illness and are unable or have difficulty attending an outpatient clinic—the elderly, debilitated, and chronically ill. Those suffering from COPD, CHF, dementia, malignancy, and chronic debility.
We are a group of providers that consist of physicians, nurse practitioners, and physician assistants. We provide medical care to patients in the hospital, post-acute rehabilitation facilities, Assisted Living Facilities, and the home.
WE ARE HIRING:
Advanced Practice Nurse for the Post-Acute Rehabilitation setting
SCHEDULE:
Monday- Friday
RESPONSIBILITIES:
- Performs initial exam and a transitional visit from the hospital to the post-acute facility
- In collaboration with the physician and other medical providers, provides and directs the treatment and manages the health conditions of patients admitted to the post-acute facility
- Counsels and educates the patients, families, and nursing staff as the need arises
- Works in collaboration with the administrative staff at the post-acute facility which may include meeting the interdisciplinary team
- Performs routine exams, orders and interprets lab data and diagnostics
- Works in collaboration with the interdisciplinary team to assist in transitioning the patient to home
- Manages chronic conditions as well as acute medical changes in the patient population at the post-acute facility
- Works in collaboration with consulting medical providers in directing treatment of medical conditions
- Working in collaboration with an interdisciplinary team to assist in supervising nursing staff
Requirements:
- Licensed Advanced Practice Nurse in the State of TN
- Active DEA license
- BLS Certification
- ACLS Certification a plus
- Experience in a hospital or post-acute setting
Job Type: Part-time
Pay: $90,000.00 - $130,000.00 per year
