Job Description:
The Community Health Worker (CHW) for The Alliance for Health Equity will be responsible for implementation of the Healthy Food as Medicine Prescription Program funded by the Jewish Healthcare Foundation. This program is an 18- month grant funded project targeting pregnant people and new families in the Greater Coatesville area. It will be a part-time position.
Job Responsibilities:
- Coordinates and participates in community events such as health fairs and community forums, and door-to-door campaigns as a representative of the program to develop relationships with potential referring community partners.
- Participates in, coordinates, and conducts training related to healthy food preparation and storage.
- Conducts community presentations and other needed outreach activities to reach the program’s targeted populations.
- Participates in training and conferences related to Healthy Food as Medicine to keep current with applicable program changes.
- Assists with the distribution of food prescriptions, food items, educational materials and other resources related to the “Healthy Food as Medicine” Prescription Program.
- Maintains record of required key performance indicators in program developed software system.
- Tracks referral data from community partners.
- Logs attendance at community events, classes, workshops.
- Adheres to HIPAA guidelines and standards for patient/client confidentiality.
- Assists Program Coordinator by obtaining resources and/or recording data to meet the goals and objectives established.
Job Description: Community Health Worker:
- Maintains inventory of healthy food education materials and equipment necessary to conduct the program.
- Conduct interviews with program participants about their experience in the Food as Medicine program.
- Participates in post teaching class focused group discussions with program participants.
- Provides telephonic follow-up to program participants as needed.
Job Requirements:
- High School diploma or GED
- 2 years’ experience in a healthcare, community-based, or social work environment
- Certified Community Health Worker, preferred
- Bilingual (English/Spanish)
- Strong verbal & written communication skills
- Good organizational skills to work efficiently and effectively
*Not required, but a plus for this unique Community Health Worker role:
- Bicultural individuals preferred
- Well-connected to the community and resources within the community
- Sees oneself as an advocate for community members and understands their challenges
- Connects program participants with resources beyond food security, including healthcare, housing, mental health services, transportation, and other social determinants of health that impact wellbeing
- A sense of humor
- Ability to be flexible and adapt to new program challenges
Technical Skills:
- Computer skills: data entry, Microsoft WORD, Excel program
- Experience with use of electronic medical record
- Experience with using Microsoft Teams (other medium) for virtual meetings
- Valid Driver’s license
- Access to a personal vehicle (travel will be a required part of this role)
- Basic CPR certification, or willing to obtain it
