Peer Bridger
Position Summary:
Under the supervision of the Director of the Peer Services Division, the Peer Bridger plays a vital role in engaging individuals residing in NYS OMH Psychiatric Centers. The Peer Bridger’s primary responsibility is to form trust-based relationships with referred individuals, foster hope for positive change and personal empowerment, and support successful immersion into the community. Additionally, the Peer Bridger models and educates both staff and referred individuals on the Peer Bridger Model, ensuring its integrity and effectiveness. This position is a paid staff role of The Alliance for Rights and Recovery.
Major Responsibilities/Activities:
- Maintain personal wellness and model healthy strategies that support individuals in their recovery.
- Provide flexible, Person First support that meets the individual needs of individuals.
- Adhere to and maintain fidelity to the Peer Bridger Model, ensuring consistent engagement with individuals residing in State Psychiatric Centers and OMH residential facilities.
- Interact with individuals from diverse backgrounds and abilities in activities or group settings.
- Organize and facilitate meetings, activities, and training sessions involving referred individuals.
- Facilitate at least 8 weekly peer support group meetings, either within the hospital setting or the local community.
- Initiate and sustain regular communication with individuals using various methods, such as face-to-face meetings, phone calls, emails, and technology.
- Support individuals in visiting meaningful community locations as part of their recovery journey.
- Continue post-discharge support to individuals, assisting them in finding and engaging with community-based support groups and activities, including employment, education, social, or spiritual interests.
- Provide regular training opportunities for staff to ensure understanding and adherence to the Peer Bridger Model.
- Maintain regular communication with the OMH Hospital Liaison and Alliance Supervisor
- Participate in required training as assigned by your supervisor.
- Report urgent matters to Supervisor/Liaison in a timely and appropriate manner.
- Keep accurate records of activities, referrals, roster, progress notes and data collection, ensuring all documentation is completed as required by The Alliance.
- Meet regularly with The Alliance supervisor to discuss progress, challenges, and program needs.
Requirements:
- NY Peer Specialist Certification (or obtained within 3 months of hire).
- Demonstrated experience and knowledge of New York State’s public mental health and substance use systems.
- Familiarity with advocacy, self-help, and empowerment programs for individuals receiving mental health and substance use services.
- Ability to inspire hope and promote recovery for individuals with psychiatric disabilities.
- Strong documentation skills, including proficiency in Microsoft Office Suite, and basic record-keeping systems.
- Strong computer knowledge and experience.
- Ability to effectively communicate both in written and oral modalities.
- Must have a reliable vehicle, a clean driving record, and adequate automobile insurance.
- Flexibility to work evenings and weekends as needed.
- Must reside in the service county.
