What are the responsibilities and job description for the MDS Coordinator - RN position at The Anderson Nursing and Rehabilitation?
Come join our family! The Anderson is currently looking to ass an additional MDS Coordinator to complement our team.
- Assist with resident admission paperwork
- Addressing the needs of residents by developing & updating comprehensive & nurse aide care plans
- Overseeing the MDS process following the RAI manual
- Work with the other MDS nurse to complete initial assessments for new admissions
- Manage patient admissions scheduling, MDS assessment & care planning
- Complete all required submissions weekly or as directed
- Review QM’s looking for ways to strengthen our program and translate to QAPI when necessary.
- Attending IDT Medicare & Medicaid meetings in collaboration with IDT to complete the MDS process
- Ensuring non-nursing sections of MDS & CAA’s accurately reflect resident needs
- Monitoring care plans to identify resident needs
- Reviewing order recap reports & nurses’ notes to ensure care plans are followed & determine whether additional OBRA MDS assessments are needed
- Monitoring residents’ change of status & determining need for change per RAI
- Ensuring accuracy of supportive documentation according to MDS/RAI regulations
- Work closely with our consultant.
- Participate in staff education to ensure proper documentation.
- Completing AIMS, Fall Risk scale, Braden, Progress note, Care Plan, nurse aide care plan paperwork
- Participate in the nursing supervisor on call schedule
Qualifications-
· Unencumbered Ohio nursing license
· Understanding of the current MDS process with the ability and willingness to learn and keep current with any changes.
· Some scheduling flexibility
· Ability to work independently
Job Type: Full-time
Pay: $80,000.00 - $100,000.00 per year
