POSITION SUMMARY: Primarily responsible for ensuring each Resident’s move and transition into their new home at The Arbors Assisted Living is a successful one. The Resident Care Coordinator will assist with completing all necessary paperwork and documentation required for new residents in accordance with established policies and procedures. The Resident Care Coordinator will assist the Case Manager with coordinating services for new residents; including but not limited to, ensuring the adjustment of the resident and family to the Community and assessing the residents’ ability to return to the Community post hospitalization/rehabilitation.
PERFORMANCE RESPONSIBILITIES:
Assists with the coordination of resident services as directed by the Executive Director.
Maintains accurate and timely documentation which complies with New York State Department of Health regulations, Arcadia Management Inc. Corporate policy and procedures and specific community practices.
Assists with completing mandated assessment tools and forms.
Obtains required pre-move in documentation (including insurance cards and prescription coverage information) and reviews same with the Case Manager and Wellness Director and/or his/her designee.
Completes move-in information with resident and/or resident’s representative; including, reviewing and signing of required forms.
Ensures residents are informed of their rights and responsibilities.
Ensures residents are informed of the Community rules.
Assists with the adjustment of the resident and family to the community.
Provides assistance and orientation to the resident during the move-in process to ensure a smooth transition.
Contacts the new resident and/or family weekly for the first month after move-in to ensure that needs are being met and that difficulties are being resolved. Documents this contact in the resident’s chart.
In conjunction with the Wellness Director and Case Manager, monitors the status of residents who are in the hospital, short-term rehabilitation, etc. Visits the Residents as directed by the Executive Director, and document the visits. Communicates from the start with the Resident’s Representative to help them through the hospital/rehabilitation process.
Maintains the Hospital Tracker in conjunction with the Case Manager & Wellness Director. Updates daily and as needed and shares the report with the Executive Director and Wellness Director on a weekly basis.
Assists with evaluating a resident’s ability to return to community prior to return from hospitalization, short-term rehabilitation, etc. Documents these evaluations and arranges for external services upon return, when applicable.
Communicates with family member/resident’s representative as needed.
Encourages resident participation in facility and community activities.
Assists each resident to maintain family and community ties and to develop new ones.
Works with all Department Heads to ensure new residents’ needs are met in their area of oversight.
Communicates resident concerns/issues with the Executive Director, Case Manager, Wellness Director, family/resident’s representative, and other departments, as needed.
Provides feedback/information for new residents for Care Team meetings.
Acts as strong clerical support to the community based on the assignment of tasks from the Executive Director.
Completes audits as directed by the Executive Director.
Maintains a positive image of the Community at all times.
