The Arc Erie County New York has been supporting children and adults with disabilities, and their families in achieving their desired quality of life by creating opportunities through comprehensive services for over 65 years.

The Arc Erie County NY is currently seeking a Licensed Practical Nurse (LPN) for our Residential program located throughout the WNY area. Our nurses in our residential care homes for adults with disabilities will provide physical care, emotional support and coordinate care for our people. This position also involves :

Assisting in monitoring and coordinating healthcare service delivery

Maintain compliance with all agency healthcare policies and procedures

Being a liaison with doctors, therapists, and other healthcare professionals

Our Full-Time positions are Monday – Friday w / no evenings, no weekends, and no holidays! Our salary range is $28-$32 per hour based on experience.

Interested candidates must be licensed and currently registered as an LPN in New York State & possess (1) yr experience working with developmentally disabled students / persons is preferred.

Job Description

Job Title :

Licensed Practical Nurse (LPN)

Department : Clinical Services

Job Grade : Position Summary :

Under supervision of an assigned Registered Nurse (RN) and in collaboration with other team members, will assist in the monitoring and coordinator of the delivery of healthcare services for the students / people we support. The LPN will assist the RN in ensuring that the evaluation, supervision, and / or provisions of nursing care and routine health maintenance programs are completed.

Reporting Relationship :

The LPN will receive general guidance and instruction from an assigned RN. The LPN will report directly to the Residential Nursing Supervisor.

Job Duties and Responsibilities :

Assists with the monitoring and coordination of the delivery of preventive routine health care, therapies and programs for individuals with developmental disabilities

Maintains compliance with and assists with ensuring facility compliance with all physician / healthcare professionals’ orders and agency policy and procedures.

Complies with all Office of People With Developmental Disabilities (OPWDD), Part 624, Part 633, and / or State Education Department (SED) regulations and reports any violations, deficiencies or incidents to supervisor

Assist with the health care needs of each student / person we support based on information available such as test results, laboratory data, and staff input

Assist in implementing an individualized plan of nursing services for each student / person who requires nursing care, including those who require medication administration for diagnosed medical conditions and update at least annually or whenever there is a significant change in the student / person’s condition

Assists in ensuring that there is adequate completion of health-related exams, appointments, laboratory / diagnostic testing as ordered and required by the RN and / or healthcare professional.

Attends medical appointments at the direction of the RN

Assists in providing initial and ongoing training to staff in all nursing procedures being delegated, including medication administration periodically reviewing that performance is consistent with standards of nursing care.

Ensure that there is specific medication information available for every medication prescribed for each student / person

Assists with the monitoring of medication administration in assigned group homes / sites.

Administers medications, treatments and supplements as assigned by the RN.

Maintains adequate first aid, medical supplies, and equipment to meet the needs of the students / persons

Completes Accident / Injury reports, and Allegations of Abuse according to regulation and agency policy

Maintains and protects confidential or other sensitive information such as Protected Health Information (PHI) and Personally Identifiable Information (PII) as defined in the agency’s information security policy.

Performs other related duties as assigned by supervisor

Job Duties and Responsibilities (Education Division Specific) :

Assist with professional nursing care to students with tracheostomies, gastrostomy / jejunostomy tubes, children with colostomies, those who may require oxygen, and children who may require intermittent catheterization in the educational program

Works with the RN to identify each student’s health needs which impacts student learning, achievement and safety

Administers prescribed medications and treatments and gathers student measurements, signs, and symptoms.

Assists in providing New York State mandated and annual screening programs (Scoliosis screening, heights, weights, vision screening)

Provides in-service training to staff and / or family members when applicable

Administers emergency first aid, or assists students in obtaining treatment at a medical facility

Consults with staff concerning health supports services for students

Counsels student and families in maintaining good health through use of proper diet, routine medical care, screening and maintaining up to date immunizations

Performs necessary written work for assigned cases including maintenance of records and preparation of reports

Job Duties and Responsibilities (Residential Division) :

Works with the RN to assess and monitor all health-related tracking forms, such as charts, Plans of Nursing Instructions, etc.

Assists in ensuring that each individual’s medication regimen is assessed at least every six months or whenever there is a significant change and provide follow-up on all recommendations.

Assess each individual’s level of capacity to administer medications at least annually.

Assist in the planning of self-administration of medication programs for individuals as needed.

Monitor implementation of special diets in cooperation with the dietitian and the Residential Habilitation Manager.

Oversee scheduling of medical appointments and hospitalizations in cooperation with the Residential Habilitation Manager and communicate with health care providers and service coordinators as needed to ensure follow through.

Job Duties and Responsibilities (Day Habilitation Services Division) :

Dispense medications as required and provide assistance with toileting, behavioral issues, meals and medical emergencies.

Address and / or report all observed or suspected situations which adversely impact on the health and safety of participants / staff.

Provide first aid as needed in cases of injury and / or illness.

Maintain and order adequate medical supplies and keep first aid items current.

Participate in interdisciplinary, six month, annual and special reviews.

Qualifications and Education Requirements :

Must be licensed and currently registered as an LPN in New York State PLUS have one year experience working with developmentally disabled students / persons.

PLUS

Valid New York State driver’s license.

Essential Job Functions :

Ability to bend, stoop, squat, kneel, sit, reach, stand, walk, push, pull, climb flight of stairs; Good physical strength and be able to lift / transfer a minimum of 50 lbs. unassisted and is able to assist in two-three person lifts as necessary; Must be able to perform physical tasks outlined in the education program’s Safe Student Movement manual and / or the agency’s Safe Person Movement Program; Must have the ability to see, hear, listen, speak clearly, good motor coordination : grip, strength, eye-hand coordination

  • Mental Requirements :
  • The ability to perform critical thinking and complex problem-solving skills, write in clear, organized manner, show good judgment, independent action, prioritizing, following directions. Works independently, as well as cooperatively, with others. Be approachable, enthusiastic and meet job-related tasks accordingly

  • Human Relations Skills :
  • The ability to communicate with individuals in a positive manner, both oral and written, serve as an appropriate role model to co-workers and program participants. Receive supervision instruction in a positive manner. Adhere to agency’s policies, procedures, rules and regulations. Maintains a professional attitude and appearance at all times.

  • Equipment Operation :
  • The ability to operate medical equipment such as nebulizers, suction devices and the like; ability to operate office equipment, such as computer, copier, fax machine, and laminator.

    In the event of a weather, health or other emergency, staff may be REDEPLOYED as needed to provide necessary staffing coverage. This may include a redeployment / temporary reassignment to another physical location or line of business, i.e. Clinicians may be redeployed to temporarily work in an IRA, a school location or Day Hab site. This may be a remote or in person assignment and will be determined based on the needs of the individuals we serve.

    This position is NON-EXEMPT from the overtime requirements of the Fair Labor Standard Act.
