Prevocational Counselor - St. Lawrence County

Position Type/Expected Hours of Work – Full-time, 40 hours per week. Typically, Monday thru Friday – 7:30am -4:00pm. Flexible to the needs of the program, may include some evening hours.

Qualifications - Bachelor’s Degree in Human Service Field. One-year experience working with the Developmentally Disabled population or in a vocational setting. Must have a valid NYS Driver’s License with an acceptable driving history per the agency’s insurance requirements

Work Authorization/Security Clearance – Authorized to work in the United States and pass all background clearances.

Summary/Objective - Responsible for each assigned Program Participant’s prevocational plan of service and required program documentation. Assist Person We Support to develop work skills and behaviors in order for them to reach their maximum prevocational potential. Provide daily interventions with the People We Support and work in cooperation with all program staff to promote Independence, Integration, Individualization and Productivity.

Essential Functions – Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

1. Complete all documentation required for annual reviews, semi-annual reviews, and interim meetings.
2. Complete Prevoc ISP programs, Habilitative Plans, monthly notes, and six-month reviews of progress on Therap. Complete T-logs and PSU data entries as necessary.
3. Complete ISP Plans and Personal Focus Worksheets as necessary in Therap.
4. Verify daily attendance records.
5. Check and monitor daily documentation that is entered in Therap by Prevocational staff.
6. Ensure that all Documentation is scanned and uploaded to Therap
7. Complete Discharge documentation and admission documentation for services.
8. Identify and help address barriers that may hinder People We Support’s progress toward supported employment.
9. Meet with individuals supported, as needed, to discuss concerns they may have.
10. Monitor waiver pre-voc services to ensure accuracy and completeness. Complete all required documentation for monthly billing and submit.
11. Assist Person We Support to gain access to needed community services and supports.
12. Maintain communication with families, care managers, residential staff and QIDPs.
13. Complete DDP1 & 2, needs assessments and annual review/revision.
14. Attend and satisfactorily complete all required in-services.
15. Assist with protective oversight.
16. Complete daily documentation as necessary for supports and goals.
17. Complete annual file reviews.
18. Coordinate, schedule and complete paperwork for annual community assessments
19. Depending on caseload, additional duties may be given.

Travel – Some travel between Jefferson and St. Lawrence Counties may be required based on program needs.

Work Environment - This job operates in a professional office environment, with some involvement in the manufacturing/warehouse area. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Noise level can be high due to the use of various machinery and equipment used on the work floor area.

Physical Demands - The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; lift; use hands to finger, handle or feel; and reach with hands and arms. Work may require lifting, pushing, or pulling in excess of 30 pounds.

Wage Range - $21.57-$24.59/Hour; More based on education and experience.
